These are 2 examples of patients with triphalangeal thumbs. The first patient has a classical Holt-Oram syndrome with cardiac defect. Note the difficulty of apposition of the thumb against the index finger.

This patient also has a triphalangeal thumb but she can appose the thumb and the index, and she does not have Holt-Oram syndrome.
