This a third trimester fetus with ductal agenesis and infrahepatic connection to the inferior vena cava. (This is the form #3 in the discussion of the various forms of ductal agenesis).

Axial and coronal views of the distended inferior vena cava:

Color Doppler demonstrate the direct flow of the umbilical vein into the inferior vena cava:

And the distended inferior vena cava (note that it is larger then the aorta):

Note that this baby also has clinodactyly in one hand...
