This is a 26-year-old woman, G2P1, with unremarkable medical history referred to our service at 19 weeks due to an anencephaly.  The first scan performed at 7 weeks was normal. The second scan was performed at 16 weeks. The risk for Down was 1/550. Nevertheless, the maternal serum fetoprotein level was high, (5 MOM).

In our antenatal unit, a scan revealed an anencephaly at 19 weeks. There were no other  associated anomalies. After extensive counseling, parents requested the termination of pregnancy. Amniocentesis was performed at 20 weeks. Karyotype was normal. An autopsy was refused after expulsion.
